Middlepath Providers are considered "in-network" through major insurance carriers including Aetna, United Health Care, and Blue Cross and Blue Shield of Illinois (PPO and Blue Choice plans) through an administrative platform called Headway.
For all other carriers, Middlepath providers are considered "out of network". If you wish, we can provide you with a superbill to give to your insurance for potential re-imbursement using your "out of network" benefits. We accept HSA, FSA, and all major credit cards. Payment is due before appointment begins.
